Nomination Submission Guidelines for Faculty and Staff Awards

Nominations for all LAS and University awards are due in the LAS College Office by Wednesday, December 16, 2009.

All nomination packets for LAS awards must include an LAS nomination cover sheet, which is available by clicking on the relevant link on the left side of this web site. The links also provide descriptions, eligibility requirements, evaluation criteria, and required nomination materials for each award, as well as the relevant LAS nomination cover sheet in both PDF and DOC format.

The nomination must be submitted electronically via email to Sue Henze (shenze@iastate.edu) in the LAS College Office by Wednesday, December 16, 2009. The electronic submission must be sent as an attached single file (either PDF or Word format) with nomination materials in the following order: (1) LAS nomination cover sheet; (2) university nomination cover sheet, if appropriate; (3) 100-word paragraph for convocation program; (4) nomination statement/letter; (5) external letters of support, if required; (6) internal letters of support, if required; (7) nominee's Position Responsibility Statement (PRS), and (8) nominee's curriculum vitae/resumé. The electronic nomination file should be named in the following manner: NOMINEELASTNAME_AWARDNAME. Please submit materials in the requested format and order to ensure timely review of the nomination.

All nominations that you wish to have considered for a parallel university award must also include a university nomination cover sheet:

  • University Faculty Awards: Word - PDF
  • University Staff Awards: Word - PDF

A single nomination may be submitted for both University and LAS awards, but the nomination must include both the relevant University and LAS cover sheets.
